Requirements

• Active RN license (Indiana)
• 1+ year clinical nursing experience
• Valid driver’s license and reliable vehicle
• Ability to travel within Fort Wayne and surrounding areas

Nice To Haves

• Home Health or Hospice experience preferred

Responsibilities

• Manage your own patient caseload
• Complete start of care and follow-up visits
• Develop and update care plans
• Communicate with physicians and care team
• Document visits accurately (HomeCare HomeBase)
• Monitor patient progress and adjust care as needed
