Requirements

• 1+ year scheduling or healthcare coordination experience
• Home health scheduling experience preferred
• Strong communication and problem-solving skills
• Ability to multitask and prioritize in a fast-paced environment
• Proficiency with Microsoft Office and ability to learn systems quickly
• High school diploma or GED required

Nice To Haves

• Homecare Homebase experience
• Bilingual Spanish/English

Responsibilities

• Schedule patient visits and manage clinician calendars
• Coordinate Start of Care, follow-ups, and recertifications
• Communicate with CTMs, clinicians, and physicians
• Adjust schedules in real time based on patient needs
• Ensure visits are completed timely and accurately
• Support workflow through Homecare Homebase and internal systems
• Balance staffing needs with patient care demands
