Career Opportunity For Home Healthcare Manager (RN) – Senior Healthcare & Health Services We are seeking a highly motivated and experienced Home Healthcare Manager (RN) to join our team at Senior Healthcare Service Company. As the Home Healthcare Manager you will oversee and manage the day-to-day operations of our home healthcare services, ensuring that our clients receive exceptional care and support.

Requirements

  • Bachelors Degree in Nursing, Healthcare Administration, or related field preferred. (Equivalent combination of education and experience will be considered)
  • Active and unrestricted California Registered Nurse (RN) License
  • Minimum of two (2) years of nursing experience in a home health setting
  • Experience working with frail, elderly, or medically complex senior populations strongly preferred
  • Supervisory or leadership experience in a healthcare environment preferred
  • Knowledge of value-based care models and interdisciplinary team (IDT) collaboration
  • Must be US Citizen or Authorized to Work in the United States
  • Direct and manage a high-performing home health department, ensuring all workflows, clinical protocols, and regulatory standards are consistently followed.
  • Recruit, onboard, train, mentor, and evaluate home health staff
  • Oversee nursing, physical therapy, and occupational therapy services delivered in participants homes
  • Ensure accurate, timely documentation and adherence to individualized care plans
  • Evaluate the quality and effectiveness of nursing assessments and IDT participation
  • Provide oversight of care delivered in skilled nursing facilities and board and care homes
  • Maintain compliance with California Department of Public Health regulations and home health standards
  • Participate in quality improvement initiatives and performance metrics monitoring
  • Collaborate closely with physicians, case managers, social workers, and community partners
  • Strong clinical judgment in managing complex geriatric conditions
  • Deep understanding of frailty, chronic disease management, and senior care best practices
  • Ability to lead, inspire, and hold teams accountable
  • Strong coaching and performance management skills
  • Excellent written and verbal communication skills
  • Proficiency in electronic health records (EHR) and documentation standards
  • Ability to manage multiple priorities in a fast-paced, field-based environment
  • Data-driven mindset with a focus on outcomes and quality metrics

  • Strong regulatory and compliance awareness

Responsibilities

  • Oversee and manage all aspects of the home healthcare services, including staffing, scheduling, and client care.
  • Develop and implement policies and procedures to ensure compliance with state and federal regulations.
  • Monitor and evaluate the performance of healthcare staff, providing guidance and support as needed.
  • Conduct regular assessments of clients' needs and develop personalized care plans.
  • Collaborate with physicians and other healthcare professionals to ensure coordinated and comprehensive care for clients.
  • Maintain accurate and up-to-date records of clients' medical history, treatments, and progress.
  • Provide training and education to staff on best practices and new developments in the home healthcare field
  • Participate in quality improvement initiatives to enhance the overall quality of care provided to clients.
