Home Hospice Intake Representative
About The Position
Responsible to execute strategic initiatives to grow and develop the business lines of Tower Health at Home. Facilitates referrals by developing relationships with referral sources, physicians, and other health care professionals. Provides the consistency and predictability necessary to transition a patient's care between care settings or levels of care within a care setting in a timely fashion. Tower Health at Home provides a comprehensive range of services that allows patients to recover from a short-term illness or manage a chronic condition in the comfort of their own home. Our team of healthcare professionals work collaboratively to deliver compassionate, high-quality care in our patient’s home for their home health or hospice needs. Requirements
- Completes and maintains all competencies and trainings as required for role.
- High School Diploma - OR - GED - OR - Relevant experience may be considered in lieu of education
