About The Position

The Home Hospice Licensed Practical Nurse (LPN) plays a critical role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses in their homes. This position focuses on managing and monitoring patients' physical symptoms, administering medications, and providing emotional support to both patients and their families. The LPN collaborates closely with interdisciplinary hospice teams to ensure comprehensive care plans are effectively implemented and adjusted as needed. By maintaining detailed clinical documentation and communicating changes in patient conditions promptly, the LPN helps optimize quality of life and comfort for hospice patients. Ultimately, this role is essential in supporting the dignity and comfort of patients during their end-of-life journey while respecting their wishes and cultural values.

Requirements

  • Current and valid WI Licensed Practical Nurse (LPN) license
  • Minimum of one year of nursing experience, preferably in hospice, palliative care, or home health settings.
  • Basic Life Support (BLS) certification from an accredited organization.
  • Strong knowledge of nursing principles, medication administration, and symptom management.
  • Ability to work independently in a home care environment and manage time effectively.
  • Familiarity with electronic medical records (EMR) systems used in home health or hospice care.
  • Excellent communication and interpersonal skills to support patients and families from diverse backgrounds.
  • Ability to handle emotionally challenging situations with empathy and professionalism.

Responsibilities

  • Provide skilled nursing care to hospice patients in their homes, including medication administration, wound care, and symptom management.
  • Assess patients’ physical and emotional conditions regularly and report any changes to the supervising Registered Nurse or physician.
  • Educate patients and family members on disease progression, medication usage, and comfort care techniques.
  • Collaborate with the interdisciplinary hospice team to develop, implement, and update individualized care plans.
  • Maintain accurate and timely clinical documentation in compliance with hospice regulations and standards.
  • Support patients and families emotionally, offering guidance and reassurance during difficult times.
  • Ensure adherence to infection control protocols and safety standards during all patient interactions.
  • Participate in team meetings and contribute to continuous quality improvement initiatives.
