Responsibilities
- Provide skilled nursing care to patients in their homes following prescribed care plans.
- Utilize Kinnser software to document patient visits and update clinical records accurately and timely.
- Assess patient condition and communicate changes or concerns to the care team promptly.
- Collaborate with interdisciplinary team members to coordinate patient care.
- Educate patients and their families on disease management, medication administration, and health maintenance.
- Ensure compliance with company policies, healthcare regulations, and professional nursing standards.
