About The Position

Admissions Registered Nurse Compassionate transitions. Strong first impression. Overview The Admissions Registered Nurse completes hospice admission assessments for patients in their homes or facility settings and supports a smooth transition into hospice services. Responsibilities include conducting initial visits, evaluating eligibility, educating patients and families, and completing timely documentation to initiate care.

Requirements

Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
Current State license as a Registered Nurse, current Drivers License
Two years experience as a Registered Nurse in a clinical care setting and 1 yr full time hospice.
Nursing skills as defined as generally accepted standards of practice.
Knowledge of State and Federal Regulations.
Good interpersonal skills.
Reliable transportation.
Valid and current auto liability insurance.
Proof of current CPR.
Ability to work flexible schedule
ability to travel locally
some exposure to unpleasant weather
PRN emergency call

Responsibilities

Complete hospice admission assessments and eligibility determinations
Educate patients and families on hospice services, philosophy, and care expectations
Coordinate admission processes with clinical and operational teams
Obtain physician orders and ensure accurate documentation
Document admission findings in the EMR (HCHB)
Participate in IDG/IDT communication as needed
