Requirements

  • Graduate of an accredited Diploma, Associate, Baccalaureate, or Masters Degree in Nursing
  • Current State License as a Registered Nurse
  • Minimum of one year nursing experience in a clinical care setting
  • Demonstrates clinical nursing competencies in accordance with accepted standards of practice
  • Able to assess and respond appropriately to the physical, emotional, and psychosocial needs of patients, families, and caregivers
  • Exhibits strong interpersonal communication skills, emotional intelligence, and the ability to support patients and families facing end-of-life issues
  • Capable of managing multiple priorities, documentation requirements, and time-sensitive responsibilities efficiently
  • Reliable transportation and valid automobile insurance coverage

Nice To Haves

  • Bachelor of Science in Nursing (BSN) preferred
  • Hospice or home health experience preferred

Responsibilities

  • Supporting patient care needs after-hours, including urgent admissions, symptom management, and other patient needs. Think of a triage/emergency room nurse for hospice patients’ after-hours in homes, facilities, and residential care communities in your neighborhood.
  • Providing positive, supportive communication to physicians, NPs, our hospice case managers, patients, and families.
