Hospice Aide - FT
About The Position
The Hospice Aide provides personal care and supportive services to hospice patients under the direction of a Registered Nurse, following the physician’s plan of care. Requirements
- Certified Home Health Aide (CHHA) – Oklahoma
- Valid driver’s license, reliable vehicle, and auto insurance
- Ability to travel within the service area
- Strong communication skills
- Comfortable working in diverse home environments
- Ability to lift up to 50 lbs
- Assist with patient transfers
- Adequate hearing and vision for patient care
Nice To Haves
- Hospice or direct patient care experience preferred
Responsibilities
- Provide personal care per the plan of care
- Assist with mobility, transfers, and ambulation
- Support patient comfort and hygiene
- Assist with meals and elimination needs
- Monitor and report changes in patient condition
- Record vital signs as ordered
- Perform light housekeeping related to patient care
- Complete accurate and timely visit documentation
- Documentation completed within 48 hours
