About The Position

Responsible for the coordination and management of the hospice bereavement program and for assisting in the delivery of grief counseling and support to patients and families.

Requirements

  • Experience or education in grief or loss counseling required.
  • Valid state driver’s license and reliable automobile, current automobile insurance, and be willing to operate personal car necessitated by nature of job.
  • Current health certificate/physical examination and TB testing.

Nice To Haves

  • Bachelor’s Degree in ministry, counseling, social work or related field preferred.
  • One (1) year experience in bereavement counseling within the last three (3) years preferred.

Responsibilities

  • Coordinates, manages, and supervises the functioning of the hospice bereavement program.
  • Makes bereavement services available to the family and other individuals in the bereavement plan of care up to 1 year following the death of the patient.
  • Functions as a member of the Hospice interdisciplinary team with responsibility for establishing, implementing, and monitoring the plans of care for bereaved family members.
  • Provides psychosocial assessment for grieving and soon to be bereaved families referred for service.
  • Helps in planning, organizing, implementing, and evaluating all bereavement functions, including, but not limited to the following: Assignment of bereavement staff and/or volunteers to bereaved family members. Referring families to appropriate community agencies based on consultation and/or coordination with other related services. Extends bereavement services to residents of an SNF/NF or ICF/IID when appropriate and are identified in the bereavement plan of care. Conducting and/or tracking bereavement assessments in an efficient and timely manner. Development, charting, and monitoring of bereavement care plan. Development of bereavement materials. Attends and participates with staff in education sessions. Staffs and monitors support groups and activities. Development and implementation of bereavement training programs. Develop bereavement evaluation instruments as needed. Provide statistical data as requested. Provides community outreach regarding bereavement education. Provides press releases and public relations on an ongoing basis notifying community of upcoming groups. Provides placement supervision of bereavement volunteers. Leads/oversees bereavement support groups. Participates in performance improvement activities.

  • Allows patient and family to voice their concerns, fears, and feelings.
  • Addresses and supports cultural practices, as long as practices do not harm others.
  • Supports a culture of safety and quality.
  • Understands hospice philosophy and issues of death and dying.
