Requirements

  • Master’s degree in Social Work (MSW) from an accredited program
  • Minimum one (1) year of healthcare experience preferred
  • Hospice or home health experience preferred
  • Knowledge of hospice philosophy and end-of-life care
  • Strong communication and organizational skills
  • Current Arizona social work licensure
  • Valid driver’s license with reliable, insured transportation

Responsibilities

  • Conduct psychosocial assessments of patients and families
  • Provide emotional support, grief counseling, and crisis intervention
  • Assist with advance directives and education on patient rights
  • Connect families with community resources
  • Participate in interdisciplinary care planning meetings
  • Maintain accurate and timely documentation
