About The Position

As a leading care provider for elders through our residential care services and community-based PACE program that keeps people connected in the community, and our grant-making foundation that supports affordable housing with services, workforce development and quality of life. As an integral member of the Hospice Interdisciplinary Group, the Hospice Social Worker is responsible for implementation of professional standards of care for medical social work hospice services. Responsible for upholding the standards, ethics, clinical practice and values of the State of Ohio CSWFMT Board, and McGregor.

Requirements

  • A graduate of a master’s program in Social Work (MSW) accredited by the Council on Social Work Education
  • Possess interpersonal skills which allows one to communicate clearly and deal tactfully with residents, families, facility staff, government agency personnel and the public.
  • Possess strong leadership ability, emotional ability, and high professional standards of care.
  • Possess knowledge of and commitment to Hospice philosophy of care.
  • Understand the grief response and experience with counseling individuals and/or groups regarding loss.
  • Utilize strong organizational skills to effectively manage caseload.

Responsibilities

  • Performs initial screening and evaluation of the psychosocial, emotional, and spiritual status of patients and families/caregivers related to the patient’s terminal illness and environment.
  • Documents assessments, visits, telephone contacts, referral actions and social service interventions in the medical record clearly and concisely in a timely manner.
  • Provides short term crisis intervention and individual or family counseling when indicated. Utilizes de-escalation techniques as appropriate. Required to be on call and available twenty-four (24) hours via telephone or in-person if an emergency visit is needed.
  • Conducts social evaluations and plans intervention based on evaluations findings. Counsels’ patient and family/caregivers as needed in relationship to issues of death and dying, stress, bereavement, and other identified coping difficulties.
  • Assists Hospice Chaplain in the development and provision of the Bereavement Program.
