About The Position

University Home Care Inc is seeking a compassionate and dedicated Hospice Registered Nurse to join our team serving patients and families in Livonia, MI, as well as Wayne, Macomb, and Oakland counties. This role involves providing exceptional nursing care to patients in their final stages of life, ensuring comfort, dignity, and support throughout their hospice journey. The ideal candidate will be committed to delivering holistic care with empathy and professionalism.

Requirements

  • Valid Registered Nursing (RN) license in the state of Michigan.
  • Minimum of 2 years nursing experience, preferably in hospice, palliative care, or community health settings.
  • Strong assessment, communication, and patient advocacy skills.
  • Ability to work independently and manage a caseload effectively across multiple counties (Wayne, Macomb, Oakland).
  • Compassionate, empathetic approach to patient and family care.
  • Reliable transportation and valid driver’s license.

Responsibilities

  • Provide direct hospice nursing care to terminally ill patients in their homes and facilities within Wayne, Macomb, Oakland counties, and surrounding areas.
  • Develop and implement individualized care plans in collaboration with interdisciplinary team members.
  • Manage symptoms and pain to maximize patient comfort and quality of life.
  • Educate and support patients and their families regarding disease progression, treatment options, and end-of-life planning.
  • Maintain accurate and timely documentation consistent with regulatory requirements and company policies.
  • Coordinate care with physicians, social workers, chaplains, and other healthcare professionals.
  • Participate in ongoing training and professional development to stay current with hospice best practices.
