Job Summary : As an On-Call RN of Kindful Health you will work with the Clinical Director to assist a care team including RNCM(s), LPN(s), Aides, MSW and Chaplain to provide best in class care to a reasonably sized case load.

Requirements

• Must be a graduate of an approved school of professional nursing.
• Must have at least 2 years of nursing experience in the Hospice service line in the state of Texas.
• The best candidate for this opportunity demonstrates mastery of all facets of hospice care and displays a communication style that makes everyone who works with them want to do their best.

Responsibilities

• Adheres to all regulatory requirements and professional standards
• Follows policies and procedures consistent with recognized standards of professional practice
• Identifies system problems or inefficiencies that may negatively affect patient care
• Manage clinical excellence within your patient census.
• Provide high levels of communication both internally with the Kindful team and externally with patients and families to assure top CAHPS scores
• Collaborate with your Clinical Director, Medical Director and entire team to provide a "concierge" level hospice experience
