About The Position

Hamilton Health Center is seeking a Housing Case Manager to support individuals experiencing opioid use disorder by promoting housing stability, recovery, and long-term well-being.

In this role, you will provide case management and care coordination services, helping clients navigate housing resources, connect to community supports, and achieve individualized recovery goals. This position is ideal for someone passionate about advocacy, community work, and making a direct impact in the lives of vulnerable populations.

Requirements

• At least 1 year of case management experience preferred
• Experience working with substance use disorder, housing services, or vulnerable populations is a plus
• Minimum of 10 years of relevant experience
• Willingness to enroll in a related Bachelor’s degree program within 6 months of hire

Nice To Haves

• Bachelor’s degree in Social Work, Psychology, Human Services, or a related field

Responsibilities

• Provide case management, care coordination, and advocacy to support client recovery and housing stability
• Develop and implement individualized service and housing plans with clients
• Connect clients to housing resources, employment support, counseling, and community services
• Maintain relationships with landlords and community partners to secure housing opportunities
• Monitor client progress and adjust service plans as needed
• Document all services and interactions accurately and in a timely manner
• Support clients in overcoming barriers to care, including transportation needs
• Collaborate with healthcare providers, agencies, and community organizations
• Participate in required meetings, trainings, and outreach activities
• Ensure compliance with confidentiality, program guidelines, and regulatory requirements
