About The Position

The essential duties of the Case Manager include but are not limited to the following activities: Provides intake, psychosocial and substance abuse assessments to and develop an appropriate treatment plan for each client in the assigned caseload. Provides case management services to assist in the creation of and adherence to individual Service Plans, identifying short-term and long-term goals and objectives to be achieved. Conducts home visits, coordinates medical, psychiatric and substance abuse services Monitors medication compliance as necessary Maintains written information or case notes regarding all contact and intervention with the clients in AWARDS/CARES system Conducts individual and group supportive counseling sessions Organizes and educates tenants through groups and other activities presenting issues of interest to the clients such as money management, employment opportunities, etc. Links clients to community services Participates in weekly clinical meetings to discuss high risk clients; updates spreadsheets related to clients’ progress Performs other duties as assigned by supervisory staff.

Requirements

  • Bachelor’s Degree in Social Work or related field of experience required
  • 1-2 years of experience working with homeless mentally ill adults or other special needs population preferred
  • Excellent oral, writing and listening skills
  • Proficiency in computer required.
