About The Position

Central Neighborhood Health Foundation is a Federally Qualified Healthcare Center committed to the Triple Aim as described by the Institute for Healthcare Improvement. Improving the US Health Care System requires simultaneous pursuit of three aims: 1) improving the experience of care, 2) improving the health of populations, and 3) reducing per capita costs of health care. Preconditions for this include the enrollment of an identified population, a commitment to universality for its members, and the existence of an organization (an "integrator") that accepts responsibility for all three aims for that population. The Housing Navigator is responsible for the development of a housing plan in collaboration with Managed Care Plan (MCP) members that will assist them with acquiring permanent housing based on their conditions and needs, providing housing-focused case management, and effectively utilizing community resources via referrals, direct interaction with landlords, and housing management on behalf of MCP members.

Requirements

  • High school diploma and some college or counseling experience in a related field
  • Experience working with individuals and/or families experiencing homelessness and case management experience or similar experience, preferred.
  • Experience providing unhoused patients or at-risk populations with housing services.
  • Must be highly motivated and a self-starter. The ability to communicate with and relate to a diverse group of people, including patients, the community, and other staff. Must have excellent administrative and organizational skills.
  • Strong interpersonal skills, cultural competence, and ability to employ sensitivity in building rapport with MCP members.
  • Ability to work independently and possess strong time management skills. Strong ability to manage time using online or paper calendars.
  • Knowledge and/or familiarity with harm reduction and motivational interviewing concepts and strategies.
  • Computer literacy required. Working knowledge of Word, Excel, and Outlook programs.
  • Familiarity with internet-based tools (webmail, Google Calendar, webtime, etc.)
  • Strong written and oral communication skills.
  • Creative problem-solving skills.
  • Valid California driver's license and legally required insurance (a CA minimum) to travel, as driving is required.

Nice To Haves

  • Previous experience assessing and referring individuals to various community resources is a plus.
  • Bilingual (Spanish/English) preferred but not required.

Responsibilities

  • Form and foster relationships with housing agencies and permanent housing providers, including supportive housing providers.
  • Partner with housing agencies and providers to offer the MCP member permanent, independent housing options, including emergency shelters, transitional, and shared living housing.
  • Connect and assist the MCP member to get available permanent housing.
  • Coordinate with MCP members in the most easily accessible setting, within MCP guidelines (e.g., could be a mobile unit that engages members on the street).
  • Conduct or review intake and assessment for each family/individual accessing services to evaluate housing needs, history, and service/support needs. Develop and implement a housing-focused individual opportunity plan in collaboration with each MCP member.
  • Participate in weekly conference calls and staff meetings.
  • Perform other related duties as assigned.
