About The Position

The Housing Placement Specialist plays a critical role in supporting individuals eligible for Recovery Bridge Housing (RBH) by coordinating housing placement, documentation, and ongoing communication between clients, treatment teams, housing providers, and community partners. Serving as a knowledgeable point of contact for RBH-related processes, the Housing Placement Specialist ensures timely eligibility verification, accurate documentation, and seamless coordination with Roots teams and community partners. This position frequently functions as an interim care coordinator for unassigned or newly referred clients, bridging clinical care, housing placement, and system navigation to support housing stability and treatment engagement.

Requirements

  • High school diploma or equivalent required.
  • Hold one of the following: Registered or Certified in Drug and Alcohol Counseling (RADT, SUDRC, CADC, etc.) Certified Peer Support Specialist
  • Valid California Driver’s License (or ability to obtain one) and current automobile insurance.
  • Background check and TB clearance as required by program standards.
  • Minimum of two (2) years of experience in housing navigation, case management, social services, or behavioral health settings preferred.

Nice To Haves

  • Associate’s or Bachelor’s degree in social services, behavioral health, human services, or a related field preferred.
  • Experience working with individuals experiencing homelessness, substance use disorders, and/or co-occurring mental health conditions strongly preferred.
  • Familiarity with Recovery Bridge Housing, Medi-Cal documentation, and coordinated care systems is highly desirable.

Responsibilities

  • Verify client eligibility for Recovery Bridge Housing using internal systems and the Housing Questionnaire.
  • Obtain, track, and follow up on all required Releases of Information (ROIs), including but not limited to partner housing providers, outpatient providers, and community partners.
  • Collect, verify, and upload required documentation, including Medi-Cal eligibility, TB clearance, and other housing-related records into internal systems and County EHR
  • Coordinate directly with partner housing programs to facilitate placement, align timelines, and respond to client-specific needs and barriers. Serve as a knowledgeable RBH point of contact for clients, peers, clinicians, admissions staff, and external community referrals.
  • Maintain accurate and up-to-date records in internal system, including Bed Referral / Bed Management boards and Admissions Leads or Potentials boards.
  • Collaborate with clinicians and admissions teams regarding current Roots clients or newly enrolled clients who are interested in RBH placement.
  • Monitor and review current RBH clients’ engagement and progress in treatment, including attendance and completion of required group hours.
  • Provide regular updates to internal teams, including during clinical team meetings, regarding RBH placements, availability, and client status.
  • Communicate housing updates across appropriate platforms, including Monday.com, PCNX, phone communication, G-chat, and in-person coordination with clinical and admissions teams.
  • Document housing-related updates in County HER for unassigned clients and ensure continuity of information across systems.
  • Connect clients with appropriate resources and guidance, including referrals to partner housing programs, care teams, supervisors, and internal Roots staff.
  • Frequently act as an interim care coordinator for unassigned RBH clients and Roots clients exploring housing options.
  • Serve as a liaison and bridge between RBH, Roots staff, housing partners, current Roots clients, and potential RBH clients.
  • Comply with all applicable program standards, contract requirements, confidentiality regulations (HIPAA), and organizational policies.
  • Embody the mission, values, and trauma-informed philosophy of the organization in all interactions with clients, staff, and community partners.
