This position provides housing-focused case management services to individuals and families at risk of eviction. The Housing – Social Worker conducts eligibility and housing stability assessments, develops individualized service plans and supports short- and long-term housing stability. The role includes ongoing case management, landlord and household mediation, coordination with legal aid and community partners and referrals to housing programs when eviction prevention is not viable. The position also assists clients with budgeting, resource planning and follow-up to support sustained housing outcomes. About Our Team Wake Prevent! is an eviction prevention program within Wake County’s Department of Housing Affordability and Community Revitalization. The program provides time-sensitive, short-term financial assistance and housing stabilization services to Wake County residents experiencing a housing crisis and at risk of eviction. Services include landlord and household mediation and ongoing case management, with a focus on early intervention to help families maintain stable housing and avoid homelessness.
