About The Position

Work is performed under the general supervision of the Home Manager with moderate latitude for use of initiative and independent judgment. Responsibilities include: Providing direct care support to those individuals in the PASRR Independent Living Skills Program by supporting goals and outcomes identified in their service plan to include transporting to community outings as well as providing Independent Living Skill services at the nursing facility. Responsible to maintain scheduling for each client to provide Independent Living Skills to include outings in the community, attending on site events with the individual at the nursing facility and or transporting to day habilitation/behavior support, according to their personalized plan and person centered preferences. Know and work to meet person centered outcomes and goals for each client receiving PASRR ILS services. Maintain close correspondence with the Habilitation Coordinator to report progress or lack of progress towards outcomes and goals. Complete all documentation and note in electronic health record within LifePath Systems required timeframe. Provide transportation for Individuals, via company vehicles and maintain status to legally operate company vehicles. (You must have a valid/current Texas Driver's License enabling you to drive at all times). Ensure the rights of the Individuals, treating them with dignity and respect. Maintain continuous supervision of all assigned Individuals in your care during your scheduled time you are providing services. Know and comply with all Company policies/guidelines per the policy and Employee Handbook/Guidebooks. Conduct all matters in a professional and mature manner while representing LifePath Systems to include interaction with all staff at the nursing facility and in the community. Attend all in-services/trainings as scheduled to meet the needs of the Individual and the LifePath Systems and maintain all Relias trainings. Communicate timely all Individual, and vehicle needs to the Program Administrator. Attend any required meetings for individuals with the nursing facility and habilitation coordinators to discuss Independent Living Skill, behavior support and day habilitation programming goals and outcomes. Encourage and support Individuals to be active in community events, outings, and opportunities. Facilitate social interaction and socially valued behavior. Give Individuals a choice of their activities as outlined in their plan of care. Comply and adhere to PASRR Rules, as well as LifePath Systems rules, policies, and procedures.

Requirements

  • High School Diploma or GED.
  • 6 months to a year of experience working with Individuals with Development Disabilities.
  • Minimum age of 18 years of age .
  • Must be a licensed driver.
  • Knowledge of human services delivery system.
  • A working knowledge of the HCS Principles, ICF Regulations and Standards as well as LifePath Systems rules, policies, and procedures.
  • The ability to communicate, orally and in writing in English.
  • The ability to organize and prioritize a variety of assignments while managing time effectively and efficiently.
  • The ability to form and maintain effective relationships with the individuals served, families, stakeholders, coworkers, and the public.
  • The ability to link plans and support activities to the outcomes identified by the individuals served.
  • The ability to document and demonstrate support activities and actions based on the outcomes identified by the individuals served.
  • Ability to solve practical problems and deal with a variety of concrete variables. Ability to interpret instructions provided in written, oral, and diagram, or schedule form.
