About The Position

The Referral and Admissions Coordinator is responsible for managing referrals and coordinating admissions into Intermediate Care Facilities (ICF/IID), DDA group homes, Adult Foster Living (AFL), and community-based services. This role works closely with North Carolina Tailored Plans, LME/MCO partners, DHHS entities, hospitals, and community providers to facilitate timely placement of individuals with intellectual/developmental disabilities and behavioral health needs. The position supports service access, regulatory compliance, and occupancy goals by maintaining active referral pipelines and ensuring all admissions processes align with North Carolina Medicaid and DHHS requirements.

Requirements

  • Bachelor’s Degree in Human Services, Social Work, Psychology, Rehabilitation, or related field required.
  • Experience working within the North Carolina behavioral health and IDD service system strongly preferred.
  • Prior experience collaborating with Tailored Plans and/or LME/MCOs preferred.
  • Knowledge of Innovations Waiver services, ICF/IID placement processes, AFL services, and NC Medicaid authorization requirements preferred.
  • Strong relationship-building, coordination, and organizational skills.
  • Ability to manage multiple referrals and respond effectively to urgent placement needs.
  • Proficiency with electronic health records and Microsoft Office applications.

Responsibilities

  • Serve as the primary point of contact for referral inquiries related to open placements in ICF/IID , DDA Group Homes , Adult Foster Living (AFL) , and community-based periodic services .
  • Coordinate and manage the full admissions process including referral review, documentation collection, clinical/programmatic match coordination, and transition planning.
  • Maintain proactive communication with North Carolina DHHS , Tailored Plans , LME/MCO care coordinators , discharge planners, hospital social workers, and other provider agencies to develop and sustain referral relationships.
  • Collaborate with Tailored Plans and LME/MCOs to support authorization processes , service eligibility determination, and funding stream alignment (e.g., Innovations Waiver, State-Funded Services, ICF Medicaid).
  • Track and report program capacity, vacancy status, and referral outcomes to leadership.
  • Participate in Person-Centered Planning discussions , transition meetings, and pre-admission screenings as required.
  • Ensure all admissions documentation meets NC Medicaid , DHHS , and internal compliance standards.
  • Conduct outreach efforts including provider networking, hospital engagement, and participation in regional system meetings to support placement development.
  • Assist in identifying service gaps and placement barriers and communicate recommendations to leadership.
