About The Position

Responsible for all areas of incident management and investigations. Will conduct investigation of incidents reported to the NYS Justice Center, OASAS, OMH, OTDA, OSHA, and DOH throughout all agency service delivery areas. Works with Directors, Managers, Compliance and Quality staff to carry out required investigation to ensure the effective delivery of services for program participants. Participate as a member of the Compliance/Quality department.

Requirements

  • Bachelor’s degree in human services or related field
  • At least 2 years related experience and/or training with Mental Health, Substance Use Disorder, Developmentally Delayed, Geriatric and co-occurring disorder clients.
  • Minimum of 2 years of experience in one or more of the following: Incident management or investigations; compliance, quality assurance, or risk management; Human services, behavioral health, healthcare, or social services; Regulatory reporting or oversight roles within NYS-licensed programs.
  • Must maintain valid driver’s license and acceptable driving record

Responsibilities

  • Impartially perform investigation activities to determine the most accurate reflection of what likely occurred.
  • Collect and analyze all available data, including, but not limited to incident reports, statements, medical records, and video footage.
  • Schedule and complete interviews with staff and patients.
  • Substantiate or unsubstantiate allegations of wrongdoing, as supported by the evidence.
  • Communicate investigation findings to Directors, Managers and Compliance/Quality Assurance staff and collaborate regarding opportunities for improvement and appropriate corrective actions.
  • Follow up with the programs to ensure corrective actions are completed.
  • Professionally correspond with the Justice Center and OASAS regarding incidents and respond to all requests within deadlines, to include closing out investigations.
  • Become proficient with incident processing within the NIMRS application in order to respond to all requests within deadlines, to include closing out investigations.
  • Accurately document and summarize each investigation. Maintain an understanding of agency and program policies.
  • Attend trainings as needed to enhance understanding of agency, program, and staff expectations.
  • Engage in classroom style Investigation Training and on the job training with seasoned investigators as needed.
  • Track all investigations assigned to and submitted by the agency. Provide status reports at least weekly and monitor for incident trends.
  • Manage data to include preparation of tables, reports, charts and graphs.
  • Participate in Incident Report Committee: create agendas, record minutes; engage in discussions; and follow up on items as needed.
  • Manage timely investigations with other investigators in order to ensure submission of all investigations by the deadline.
  • Understand the agency incident policies in order to identify additional incidents or when to cease an investigation and re-report it.
  • Be available to work in all counties where services are provided at the hours necessary for investigation purposes.
  • Directly identifies omissions of job performance for timely coordination with supervisors.
  • Perform other duties as assigned or that may develop.
  • Management has the right to add or change the duties of this position at any time.
