Job Summary The Independent Living Case Manager role is a direct services position and is responsible for providing case management services for young adults age 18 to 23 who are in one of the Independent Living (IL) Programs include Extended Foster Care (EFC), Postsecondary Education Services and Support (PESS) and Aftercare (AFC). Hours may be untypical of the traditional 8:00-5:00 standard business hours depending on the needs of the young adult and the issue that may need to be handled after hours. This position is designed to assist young adults in reaching principal outcome areas as they move toward self-sufficiency. The Case Manager is responsible for the examination of the programs while maintaining focus on the following federal outcome areas: increasing financial self-sufficiency/employment opportunities, educational attainment, increasing connections to caring adults, reducing homelessness, reducing high-risk behavior, and improving access to health insurance. The IL Case Manager will also be responsible for ensuring a proper transition for children turning 18 to ensure they are prepared to qualify one of the IL programs.
