Job Summary: The Case Manager is responsible for coordinating and overseeing individual and group case management services and support provided to individuals struggling with behavioral health issues and addiction.

Requirements

  • High School Diploma or GED required, Bachelor's degree or high in Human Services preferred.
  • CDCA license and QMHS III certification required.
  • CPR certification required, will train upon hire.
  • Must have a valid driver's license, acceptable driving record, be 21 years or older, and be deemed insurable through Team Recovery's insurance carrier.
  • 3 or more years of experience in addiction recovery or behavioral health settings, with a strong understanding of case management and community resource connections.
  • Communication and interpersonal skills.
  • Ability to coordinate with multiple different departments professionally and respectfully.
  • Commitment to ongoing professional development and knowledge of evidence-based best practices in behavioral health.
  • Commitment to cultural competence, diversity, equity, and inclusion in clinical practice and program development.
  • Strong analytical skills and desire to drive innovation.
  • Ability to read, write, & speak clearly and listen for understanding and clarity.
  • Comfortable using technology in the workplace including Google, Microsoft, etc.
  • Prior knowledge or use of Electronic Medical Records.
  • Lived experience or knowledge or addiction and mental health disorders and how it impacts the individual and family.
  • Working knowledge of community resources available to clients and referral processes.
  • Ability to remain calm in emergent situations.
  • Flexibility to adapt schedule and assumption of duties not outlined in the official job description which are related to work as a member of a team who treats addiction.

Nice To Haves

  • 1 year of Case Management experience preferred.

Responsibilities

  • Conduct assessments to determine the needs of clients seeking behavioral health and addiction recovery services.
  • Develop individualized treatment plans in collaboration with clients, healthcare providers, and other stakeholders.
  • Coordinate care and services provided to clients, including therapy, medication management, and group therapy.
  • Monitor and evaluate clients' progress towards treatment goals and make adjustments to the treatment plan as needed.
  • Advocate for clients to ensure they receive appropriate care and support from healthcare providers and community resources.
  • Provide education and support to clients and their families about behavioral health issues, addiction recovery, and coping strategies.
  • Maintain accurate and up-to-date documentation of client interactions, treatment plans, and progress reports.
  • Collaborate with a multidisciplinary team of healthcare professionals, including clinical and medical teams.
  • Stay informed about current trends and best practices in behavioral health and addiction recovery services.
