The Patient Educator (RN) provides individualized nursing education to patients and families using an evidenced-based, multi-disciplinary approach. This role ensures the patient has the knowledge to care for his/her condition and is educated and empowered to participate in the plan of care. Serves as a liaison among clinical service staff, physicians, education departments, and patients. Provides health assessments, plans and reassesses as well as makes appropriate referrals for outside services/continued care. Participates in data collection and analysis of clinical outcomes. Assists with education of clinical professionals. Keeps current in best practices by attending conferences, educational programs and continuing education classes. Functions both in inpatient and outpatient environments. May develop education programs and facilitate educational classes. Performs other duties as assigned. INPATIENT EDUCATOR UNDER THE GUIDANCE OF MARY AND DICK ALLEN DIABETES CENTER: As an Inpatient Diabetes Educator, utilizes clinical expertise to identify educational needs of patients who are newly diagnosed with diabetes. Demonstrates the ability to work independently in a self-directed manner to provide diabetes education and support. Performs learning needs assessments using the ADCES7TM framework. Initiates diabetes self-management. Educates patients and family about hyperglycemia and its management. Ensures close collaboration with Mary and Dick Allen Diabetes Center, hospital personnel, and discharging patient for outpatient diabetes self-management and training. Documents status of diabetes education and communicates diabetes-related patient needs to other members of the healthcare team. Provides nursing care in accordance with professional, regulatory standards and evidenced based practice. Maintains standards of nursing care as recommended by the Nursing Departments philosophy and objectives. Supports the hospital staff in the implementation of “management of inpatient hyperglycemia.”
