About The Position

University of Washington Medical Center-Montlake has an outstanding opportunity for an experienced hospice nurse to join our organization. The Inpatient Hospice Nurse partners with disciplines and teams across the hospital, focusing on a goal of improving access to hospice services for hospital-based and emergency room patients that are eligible for hospice.

Requirements

  • Bachelor's of Science in Nursing (BSN)
  • Working knowledge of hospice care, hospice eligibility, and end of life care
  • Leadership skills, well-developed communication skills, and clinical expertise in the care of patients with a life-limiting illness required
  • Experience in hospice case management or hospice nursing required; case management or discharge planning experience preferred
  • Current license to practice as a registered nurse in the State of Washington
  • Active BLS certification

  • Experience in hospice case management or hospice nursing required; case management or discharge planning experience preferred

Responsibilities

  • Identifies hospice appropriateness by clinical evaluation and examination while applying expert knowledge of routine and general inpatient hospice criteria.
  • Works collaboratively and attends interdisciplinary rounding with palliative care and critical teams to identify patients in need of hospice education, access and care.
  • Identifies patients that teams feel may be appropriate for hospice and reviews patients with a new comfort care order set.
  • Develops and leads training sessions for teams/services on the topic of hospice, end of life care, and regulatory requirements, among other associated topics.
  • Serve as point of contact for hospice clinical intake/admission staff or liaisons for clinical questions that may impact acceptance to hospice determinations.
  • Identifies barriers and improves processes for patients referred to hospice in the community who did not enroll.
  • Evaluates and assesses hospice related metrics and related data to help formulate additional necessary interventions and process improvement initiatives.
