About The Position

The Intake and Diversion Coordinator plays a critical role in supporting families experiencing homelessness by serving as the first point of contact for the shelter. This position is responsible for managing the intake and exit process, assessing eligibility, providing shelter diversions services and coordinating with shelter staff.

Requirements

  • A bachelor's degree in human services or a related field, or commensurate experience
  • Three or more years of experience in the human services field, including mental health, substance abuse, homelessness, or domestic violence
  • Proven ability to adapt and remain flexible in accordance with county guidelines
  • Strong verbal and written communication skills
  • Willingness to work evenings and weekends as needed
  • Leadership and management experience is a plus

Nice To Haves

  • Bilingual English and Spanish preferred
  • Experience working in programs serving homeless families
  • Experience working in a residential environment
  • Current CPR and First Aid certification

Responsibilities

  • Conduct initial screenings and assessments to determine eligibility for shelter services
  • During intake and screening, support clients in exploring and securing viable housing alternatives such as staying with friends or family as part of shelter diversion efforts
  • Collect and enter accurate client information into the HMIS (Homeless Management Information System) or other databases
  • Provide timely updates on the status of referrals in HMIS
  • Maintain and update Client Name List
  • Conduct orientations to explain shelter rules, expectations, and services available to families
  • Coordinate move-in dates and ensure families are oriented upon entry
  • Work closely with case managers to share assessment information and support service planning
  • Schedule and facilitate intake meetings
  • Connect client with Residential Coordinators after intake is completed
  • Generate reports and maintain compliance with agency, funder, and HMIS requirements
  • Once a client exits, ensure all data is updated in HMIS database and that files are closed out and filed away appropriately
  • Participate in team meetings, trainings, and coordinated entry discussions
  • Support shelter operations as needed, including crisis response or coverage for front desk
  • Attends meetings such as Intake Coordinator meeting
