About The Position

Care Coordinators are responsible for gathering, preparing, updating, and maintaining client paperwork and ensuring eligibility for services. Care Coordinators need to assess needs, develop, coordinate, monitor, and update all services for the client. Care Coordinators are responsible for collaborating with the billing team/assisting with gathering information for billing purposes and investigating specific client issues associated with internal documentation and external requirements from agencies such as Medicaid or the PASSE.

Requirements

  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write detailed reports and correspondences. Demonstrates both verbal and written communication skills.
  • Skilled in operating computer systems and programs i.e., Microsoft Word, Outlook, and Excel, and the ability to prepare documents; interpret and develop spreadsheets.
  • Knowledge of policies, procedures, and regulations for ERC, DPSQA, Medicaid, and other affiliated client care procedures and policies required for billing and coordination of care.
  • Knowledgeable of adult learning training methods.
  • Ability to deliver a variety of training methods.
  • Ability to prepare training material and packets, as needed.
  • Ability to communicate effectively and timely with clients (and guardians), the general public, colleagues, and supervisors.
  • Ability to read and interpret documents, reports, or manuals.
  • Minimum level of education required – High School Diploma, GED, or equivalent
  • Minimum years of experience required – Two years of case management experience.
  • Proficiency in Microsoft Office Suite required.
  • Must pass an extensive background check.
  • Must comply with drug testing requirements.
  • Reliable transportation is required.
  • If you are a van driver for ERC, you must have a valid Arkansas driver’s license, meet safe driver’s requirements, and be 18 years of age.
  • Licensure requirements- Must meet minimum licensure annual training requirements.
  • This role includes licensure requirements for Administrators, Health & Safety, and HIPAA training.

Nice To Haves

  • Preferred level of education required – Trade School or Bachelor’s degree in a related field.
  • Preferred years of experience required – Three or more years of working with individuals with disabilities.
  • Preferred knowledge of programs and services for individuals with disabilities.
  • Preferred experience working with nonprofit organizations.
  • Previous work with state licensures and related annual audits a plus.
  • Required and or Preferred Certificates for this job – CCMC certified preferred.

Responsibilities

  • Develops and maintains positive working relationships with area businesses by increasing the awareness of services provided by ERC. (Such as Medical offices, State entities, and other organizations associated with client care.)
  • Responsibilities could/may include managing the Caretaker Home Assistance Program (CHAP). This includes ensuring that all contractors are compliant with the terms of their contract and ERC staff is providing the oversight necessary to ensure program success.
  • Gathers and maintains all Medicaid, PASSE, and personal enrollments/paperwork during an intake and throughout the client’s time with ERC.
  • Develops, gathers, and maintains the prior authorizations, PCSP, scripts, IPPs, behavior support plans, and other service paperwork for clients.
  • Effectively plans client care to ensure no disruption of services.
  • Enters and maintains client profiles in the Electronic Medical Records (EMR) and related software.
  • Resolves issues with Medicaid, PASSEs, or other entities when there is an issue brought to the attention of the Care Coordinator.
  • Collaborating with the billing department of consulting services provided.
  • Assesses the needs of the client and assists with the development of plans provided by the PASSE.
  • Maintains documentation and files in accordance with DDS, DPSQA, and Medicaid requirements and Elizabeth Richardson Center policies and procedures.
  • Assure clients participate in and are informed of progress toward goals and objectives as written in the individual's program plan. (Quarterly Reports)
  • Makes and bills for referrals for other services when appropriate.
  • Assure clients’ rights and safety while treating others with dignity and respect in all situations as required by the Elizabeth Richardson Center Policies & Procedures and State/Federal guidelines.
  • Coordinates and facilitates additional interdisciplinary meetings as needed to address programming issues.
