About The Position

The Intake Clinician is responsible for conducting comprehensive clinical assessments for individuals seeking services at Phoenix Behavioral Health. This role serves as the first clinical point of contact for potential clients and plays a critical role in determining appropriate levels of care, facilitating admissions, and ensuring all required clinical documentation is completed in compliance with agency policy, state regulations, and Joint Commission standards. The Intake Clinician collaborates closely with the admissions, clinical, and medical teams to ensure timely access to treatment and a smooth transition into programming.

  • Must have current LSW, LSCW, LAC or LPC

Responsibilities

  • Conduct comprehensive biopsychosocial assessments for individuals seeking behavioral health or substance use treatment services.
  • Evaluate presenting concerns, psychiatric history, substance use history, medical conditions, and risk factors.
  • Determine appropriate level of care and eligibility for program admission.
  • Coordinate with the clinical team regarding admission decisions and treatment needs.
  • Facilitate intake appointments either in person or via telehealth.
  • Complete all required clinical documentation including: Biopsychosocial assessments Risk assessments Initial treatment recommendations Admission paperwork
  • Ensure documentation meets regulatory standards including Joint Commission, state licensing requirements, and agency policies.
  • Maintain accurate and timely entries in the electronic health record (EHR).
  • Assess clients for suicide risk, homicidal ideation, and other safety concerns.
  • Follow agency protocols when higher levels of care or emergency intervention are required.
  • Coordinate crisis response when necessary.
  • Communicate admission outcomes with the admissions team, therapists, and medical providers.
  • Provide hand-off information to the assigned primary therapist and treatment team.
  • Coordinate with referral sources, hospitals, and external providers as needed.
  • Provide clear information to prospective clients regarding program expectations, services, and next steps.
  • Ensure clients feel supported and informed during the intake and admission process.
