About The Position

New Horizons, a leading nonprofit dedicated to empowering individuals with special needs, is seeking a proactive and motivated Intake Coordinator to join our team. In this role as an Intake Coordinator, you will play a vital part in supporting our Employment Services team, ensuring an exceptional level of customer service. You will collaborate closely with our Member Engagement and Resource Specialist, helping to streamline the intake process and create a welcoming, supportive experience for those we serve. Full Time - Non-Exempt Pay Rate: $22-$24 SUMMARY: The ES Intake Coordinator is an essential part of the Employment Services department as it is the “face” of the agency for Members interested in joining the ES department, as well as working with our referral partners and other key stakeholders.

Requirements

  • Must demonstrate the ability to multitask and organize work in an efficient, timely and accurate manner to meet the requirements of the job.
  • Computer proficiency, including Word, Excel, and database management required.
  • A pleasant telephone and online personality, and the ability to interact appropriately with the public are required.
  • The individual must have a reliable vehicle, proof of vehicle insurance, a valid California Driver License, and a driving record acceptable to the agency’s insurance carrier.
  • Must be able to pass a criminal background check.
  • Bachelor’s degree or a minimum of two years’ work experience with adults with developmental disabilities is required.
  • Ability to read, write and interpret proficiently in English.
  • Ability to write and comprehend documents in English.
  • Ability to compose professional correspondence.
  • Ability to effectively present information in English and respond to questions in English from groups or individuals.

Nice To Haves

  • Some customer service or database management experience is preferred.
  • Bilingual (English/Spanish) skills are preferred.

Responsibilities

  • Works closely with Member Engagement and Resource Specialist, the ES Director, and other staff in the ES department to recruit, retain and bill for services related to intake and retention services.
  • Coordinates and conducts Supported Employment, Pathways, Direct Placement and External Situational Assessment from Department of Rehab (DOR), Regional Center (RC), private pay, or other resources.
  • Schedules and follows up bi-weekly meeting with DOR counselors. Responsible for coordinating and writing final report for all the Department of Rehab External Situation Assessments.
  • Completes all required Department of Rehab paperwork for Supported Employment, Pathways, Direct Placement and External Situational Assessment as required in a timely, professional manner.
  • Responds to initial intakes within 24-48 hours.
  • Responsible for helping Members develop ISP goals and update resumes, cover letters and other employment-related documents.
  • Responsible for following up with billing, case closures, etc., with DOR and RC, including milestone billing and payments, including “26” closures, CIE, PIP and other billing related milestones.
  • Provides monthly productivity report to external partners and family members as needed or required.
  • Tracks the effective date of authorization for Direct Placement services, and follows up if/when there are issues.
  • Responsible for end-of-the-month Direct Placement, ESA and other billing oversight assigned.
  • Facilitates Job Club as needed.
  • Provides additional support to the Employment Services Director as needed.
  • Maintains case records up to CARF and other funder standards.
  • Acts as a substitute Job Coach as needed.
  • Supports Employment Services Team members as needed to ensure the highest level of customer service.
  • Serves as backup to the agency Member Engagement and Resource Specialist in area of intake.
  • Maintains the highest professional and ethical standards at all times.
  • Attends meetings and trainings as directed by the Employment Services Director.
  • Completes all paperwork, documentation, or progress notes timely and accurately.
  • Maintains security and confidentiality of records, files, database, and offices within jurisdiction.
  • Supports the Agency’s mission and partners with others to accomplish the mission.
  • Performs other duties as assigned.
