Position Overview: The Intake & Eligibility Coordinator serves as a first point of contact for clients throughout the referral and intake process. This position will focus on working with clients seeking long-term care benefits and is central to Lifelong’s role as a Beneficiary Service Provider for WA Cares Fund that provides working Washingtonians with access to long-term care benefits. This position is responsible for handling new referrals, screening clients for program and benefit eligibility, and referring eligible clients to Care Coordinators to complete assessment and access benefits. The Intake & Eligibility Coordinator is a hybrid position reporting to the Care Coordination Manager. This is a regular, full-time, non-exempt, union position based in Seattle with responsibilities at Lifelong’s Georgetown Yards and Capitol Hill offices, and in the field to meet client or program needs.

Requirements

  • Proficiency in Microsoft Office Suite (especially Excel); familiarity with Access, Word, PowerPoint, and Outlook.
  • Experience with data entry and electronic document management with a high degree of accuracy.
  • Strong organizational skills and the ability to prioritize multiple tasks with minimal supervision.

  • Clear and professional verbal and written communication skills; excellent customer service and phone etiquette.

  • Associate's or Bachelor’s Degree in a relevant field or equivalent working experience and two years of experience in an information and navigation support, administrative, or eligibility support role in a healthcare or social service setting, or Three years of paid, full time, social/human/customer service experience in information and navigation supports to vulnerable populations such as children and families, older adults, adults with disabilities and caregivers, and a high school diploma or its equivalent,
  • Experience working with vulnerable populations such as children and families, older adults, adults with disabilities and caregivers.

Responsibilities

  • Intake and Application Coordination Manage client intakes from start to finish, including monitoring incoming referrals and requests for services, conducting intakes, assessing eligibility, gathering necessary documentation, creating client profiles, and processing applications to facilitate smooth and timely service delivery.
  • Guide clients through intake process, prepare them for assessment, and complete warm handoffs to Care Coordinators to facilitate smooth and timely service delivery.
  • Assist clients with benefits access systems navigation.
  • Provide logistical, organizational, and administrative support by coordinating all aspects of incoming WA Cares referrals.
  • Process applications in a timely manner.
  • Customer Service Communicate professionally with clients, internal teams, and community partners via phone, email, and in-person interactions.
  • Utilize de-escalation skills as necessary in working directly with clients.
  • Understand and apply foundational knowledge of the WA Cares Fund to support beneficiaries, uphold program policies, and represent the WA Cares culture through compassionate, informed service.
  • Provide benefit navigation and customer support and assist clients and staff in gathering required intake and eligibility documentation.
  • Utilize a multifaceted, client-centered approach to outreach, incorporating phone calls, in-person meetings, mailings, and collaboration with external care providers, to ensure seamless enrollment and ongoing access to services.
  • Data Entry and Integrity Perform a high-volume of data entry with strong attention to detail.
  • Tasks include creating and updating client profiles, scanning documents, generating reports, managing multiple databases, processing mail merges, and ensuring accuracy and consistency across systems.
  • Research, Analysis, and Quality Improvement Use critical thinking and data analysis to identify trends and improve program operations.
  • Conduct both routine and ad-hoc research to assess process effectiveness and recommend improvements to enhance the intake and eligibility experience.
  • Program Coordination Partner with the Care Coordination Manager to implement and refine intake-related policies and procedures.
  • Build and maintain collaborative relationships with internal departments and external care partners to support seamless client transitions and continuity of care.
  • Work together with Care Coordinators and other ADS staff to provide quality service delivery and beneficiary access support.
  • Organizational Responsibilities Participate in team meetings, all-staff trainings, and Lifelong events.
  • Build knowledge of aging and disability services and other chronic health supports, as well as the services offered by Lifelong.
  • Work respectfully with clients, staff, and volunteers from diverse backgrounds.
  • Maintain professional boundaries, confidentiality, and consistent, reliable working hours.
  • Engage in Lifelong’s Employee Performance Management Process.
