JOB SUMMARY Under the direction of the Integrated Behavioral Health Manager, IBH Care Coordinator will have three essential duties. First, the care coordinator will be responsible for supporting the day-to-day operations of the IBH team by interfacing with IBH behavioral health consultants, medical providers and patients presenting to primary care for a range of behavioral health conditions including diabetes, hypertension, depressive and anxiety disorders, and substance misuse. Care coordinator will assist in identifying patients who would benefit from behavioral health consultation and ensure that patients receive the services that they need. Second, the care coordinator will link patients to needed resources in the community and assist them in overcoming barriers to access, including helping patient navigate the system and completing necessary forms and applications as needed. Finally, the care coordinator will assist the IBH team in providing measurement-based patient centered care examining interventions and patient data from the Indian Health Board's electronic medical record.

Requirements

  • Bachelor's degree in public health, community health, psychology, social work or a related field.
  • Preference for experience and knowledge working with healthcare data to examine process and outcome.
  • Strong written and verbal communication as well as interpersonal skills to work collaboratively with both patients and clinic staff.
  • Must have interest in gaining experience and knowledge in trauma informed care.
  • Knowledge and respect of the American Indian community.

  • Familiarity and engagement with the Minneapolis Urban Indian community is a plus.
  • Preference for experience working in a multidisciplinary team.

Responsibilities

  • Scrub schedule daily for behavioral health providers to identify patients needing BHC services.
  • Create, develop and maintain patient educational handouts and resource list.
  • Link patients to needed resources in the community and assist them in overcoming barriers to access, including helping patient navigate the system and completing necessary forms and applications as needed.
  • Community outreach and recruitment for various IBH and IHB groups and programming as needed.
  • Assist with annualized events that focus on health and wellness and IHB promotion.
  • Develop and maintain patient registries that can be utilized in measurement-based care to track and adjust patient treatment as well as aggregate data for tracking practice performance on process and outcome measures.
  • Generate weekly audit reports such as behavioral health utilizations/visits and report on administration of mental health screening tools during primary care visits.
