About the Project/Clinic: AMI will be assisting in providing medical staff to the Charlie Norwood VA Medical Center in Augusta, GA to help deliver compassionate and quality healthcare to patients in the area. Job Summary: AMI is seeking a skilled and dedicated Primary Care Physician to provide comprehensive medical care to eligible patients at the Charlie Norwood VA Medical Center. The physician will deliver preventive, chronic, and acute care services while working as part of an interdisciplinary team within the Patient Aligned Care Team (PACT) model.

  • Must be able to successfully complete the AMI Credentialing Process.
  • Proficiency in Microsoft Office Suite applications
  • MD/DO degree from accredited institution
  • Active/unrestricted license to practice
  • Proof of board certification
  • Must possess/obtain a National Provider Identifier
  • Must possess valid DEA registration
  • Active BLS/ACLS
  • Must be able to work in a safe manner following all safety rules and regulations.

  • Utilize appropriate methods for interacting effectively and professionally with persons of all ages and from diverse cultural socioeconomic, educational, racial, and ethnic, sexual orientations, lifestyles, and physical abilities.
  • Deliver targeted, culturally appropriate information to help individuals and groups understand health promotion and disease prevention information, policies, regulations, and local code.

Responsibilities

  • Provide comprehensive primary care services including preventive, acute, and chronic disease management.
  • Diagnose and treat common medical conditions such as diabetes, hypertension, COPD, heart disease, and other chronic illnesses.
  • Conduct patient evaluations, physical examinations, and diagnostic assessments.
  • Order and interpret laboratory tests, imaging studies, and other diagnostic procedures.
  • Prescribe medications and develop treatment plans consistent with evidence-based medical practices.
  • Provide health promotion counseling, including lifestyle modification, smoking cessation, nutrition, and exercise.
  • Screen for mental health conditions such as depression, PTSD, and substance use disorders and coordinate referrals when appropriate.
  • Address geriatric care needs including cognitive decline, fall risk, and polypharmacy.
  • Serve as a team leader within the Patient Aligned Care Team (PACT) model.
  • Coordinate patient care with specialists, pharmacists, social workers, and other healthcare professionals.
  • Manage referrals, consultations, and follow-up care for patients requiring specialty services.
  • Collaborate with community providers when appropriate through VA care networks.
  • Conduct telehealth visits, telephone consultations, and secure messaging interactions.
  • Utilize remote patient monitoring tools when available.
  • Promote patient engagement and participation in treatment plans.
  • Maintain accurate and timely documentation in the VA electronic health record system.
  • Ensure medical records meet applicable regulatory and documentation standards.
  • Comply with VA policies, clinical guidelines, and quality performance measures.
  • Communicate test results to patients and providers in accordance with established protocols.
  • Participate in staff meetings, quality improvement initiatives, and performance evaluations.
  • Complete quality assurance and performance improvement documentation.
  • Maintain compliance with required training, continuing medical education, and credentialing standards.
