Requirements

  • High School Diploma or equivalent.
  • Experience supporting individuals with intellectual and developmental disabilities preferred.
  • NYS valid driver’s license and reliable transportation required.
  • Strong communication, interpersonal, and problem-solving skills.
  • Ability to work independently, manage schedules, and adapt to different work environments.
  • Commitment to fostering inclusive, integrated employment opportunities.
  • Proficiency in using smartphones and workplace software (e.g., Microsoft Office)
  • Must possess and maintain a personal cell phone for work-related communication; this is a condition of employment.

Responsibilities

  • Provide on-site and off-site support to participants to ensure successful adjustment to their workplace.
  • Assist participants in learning essential job tasks, workplace routines, and expectations.
  • Support the development of soft skills such as communication, teamwork, professionalism, and problem-solving.
  • Collaborate with employers, supervisors, and coworkers to promote natural supports and inclusion.
  • Monitor participant progress, documenting performance, barriers, and growth in accordance with OPWDD and agency guidelines.
  • Facilitate travel training and assist with transportation coordination as needed.
  • Provide job retention support, including ongoing follow-ups, to ensure participants maintain and thrive in their roles.
  • Assist in developing individualized strategies for participants to overcome challenges and achieve greater independence at work.
  • Participate in person-centered planning, life plan and team meetings, offering input on vocational goals and progress.
  • Write clear, timely, and detailed daily and monthly notes summarizing interactions, supports provided, and participant outcomes.
  • Maintain compliance with all documentation, billing, and confidentiality requirements.
  • Promote advocacy, self-determination, and empowerment of all participants.
