About The Position

The Lead Case Manager oversees the day-to-day operations of the housing program and ensures all program activities are implemented in compliance with HUD regulations, contract requirements, and organizational policies. This role functions as the primary program operations lead, responsible for coordinating service delivery, monitoring unit and program compliance, managing reporting requirements, resolving client-related issues and maintaining a caseload.

Requirements

  • Graduation with a two-year degree in Psychology, Social Work, Community and Human Services, Rehabilitation Counseling or a related field is preferred; and High School diploma or equivalent is required.
  • Two (2) Years Case Management in Permanent Supportive Housing setting preferred
  • Excellent communication skills
  • Good computer skills
  • Good documentation skills
  • Be able to multi-task
  • Valid driver’s license
  • Must maintain and execute confidential information
  • Demonstrated ability to work with diverse communities.
  • CPR Certification, First Aid Certification, and an Annual Tuberculosis test

Nice To Haves

  • Two (2) Years Case Management in Permanent Supportive Housing setting preferred
  • Graduation with a two-year degree in Psychology, Social Work, Community and Human Services, Rehabilitation Counseling or a related field is preferred

Responsibilities

  • Oversight of day-to day operations of the program
  • Intake and assessment of incoming participants to determine eligibility and appropriate services.
  • Maintain a caseload of 7 Families/Individuals.
  • Make referrals and appointments with appropriate agency, services and/or case managers.
  • Maintain client files according to company policy and contract requirements.
  • Conduct regular program file audits to ensure compliance with HUD, contract, and organizational documentation requirements.
  • Oversee unit compliance and ensure all annual inspections are completed timely and in full compliance with HUD standards.
  • Heavy documentation; HMIS data entry
  • Prepare weekly, monthly and annual reports.
  • Maintain relationships with referring and supporting agencies and represent the agency at various community meetings as requested.
  • Provide all services in linguistically and culturally appropriate manner, including use of interpreters for those with limited English proficiency
  • Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and
  • Perform any other duties as assigned.
