About The Position

Leads, plans and conducts nutritional programs to assist in the promotion of health and control of disease. Provides nutritional assessment, education and counseling to individuals and/or groups. Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility. Oversees the assessment of nutritional needs and provides nutritional counseling to assigned clients/patients. Considers patients' cultural, educational, and learning issues. Plans individualized nutrition education to patients, their families, and/or significant others based on their needs assessment. Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility. Ensures communication of patient information to referring providers and appropriate staff in a timely manner. Ensures documentation of nutrition information discussed, written materials provided, and evaluation of patient’s receptiveness and comprehension of the education provided. Participates in professional meetings and conferences. Performs related duties as required. Requirements

  • Masters Degree in Food, Nutrition or Dietetics, required .
  • Registered Dietitian, required.
  • OR Bachelors Degreein Nursing, required .
  • Current certifcation to practice as a Registered Dietitian in New York State, (CDN) required.
  • Current Diabetes care and Education Specialist Certification (CDCES) Certification, required.
  • Minimum of five(5) years progressively responsible, related experience to ensure familiarity with scope of program.
