About The Position

Alluvion Health, a mission-driven Federally Qualified Health Center (FQHC), is seeking a dedicated Professional Counselor Licensed Candidate (PCLC), Licensed Clinical Professional Counselor (LCPC),  Licensed Master Social Worker (LMSW), or Licensed Clinical Social Worker (LCSW) to join our school-based behavioral health team. You'll play a critical role in improving children and youth mental health, increasing access to care, and building wellness within our community. What You’ll Do: Collaborate with primary care providers and behavioral health team members to assess, treat, and monitor patients with mental health and/or substance use concerns. Conduct DSM-5-aligned evaluations and use measurement-based tools (e.g., PHQ-9) to track symptoms, treatment response, and side effects. Support psychotropic medication management by monitoring adherence and reporting concerns to providers. Coordinate referrals to in-house and external evidence-based treatments and social support services. Maintain accurate and timely documentation in the electronic medical record system. Engage patients in ongoing care and evaluate progress through regular follow-ups and team consultations. Participate in quality improvement initiatives, peer review, meetings, and committee work as assigned. Uphold HIPAA standards and Alluvion Health policies while contributing to a collaborative, mission-driven care environment. Perform other related duties as assigned. Requirements

  • Current Montana license as a PCLC, LCPC, LMSW, or LCSW, or the ability to obtain one before starting, required
