We are seeking a Licensed Master of Social Work for evaluations, nursing guidance, and monthly chart reviews of ventilation patients we are currently servicing. Requirements
- NY State LMSW license
- 2 years working as a social worker in a clinical environment.
- Must be able to lift 50lbs.
- Must be able to stand/work for extended periods.
Nice To Haves
- Experience working in Home Care as a Social Worker preferred.
Responsibilities
- Assess client home environment, factors impacting provision or barriers to client’s care.
- Make recommendations for community and other resources for clients and family which facilitate the established plan of care.
- Participate in client conferences as required.
- Must be able to lift 50lbs.
- Must be able to stand/work for extended periods.
- Additional responsibilities as required.
