About The Position

"Navitas Healthcare, LLC" is seeking a Licensed Master Social Worker (LMSW) to provide comprehensive social work services in a hospital-based inpatient setting. The ideal candidate will have experience in medical/surgical, rehabilitation, or hospital environments and will work collaboratively with interdisciplinary care teams to support patient care.

Requirements

• Master’s Degree in Social Work
• Licensed Master Social Worker (LMSW) in New York State (Primary Source Verification required)
• Minimum of 1 year of LMSW experience in a hospital or healthcare setting

Responsibilities

• Conduct psychosocial assessments and coordinate discharge planning for inpatient patients
• Support patients and families with care coordination, resource planning, and counseling
• Collaborate closely with physicians, nurses, and other members of the multidisciplinary team
• Assist with crisis intervention and provide patient/family counseling as needed
• Maintain accurate and timely documentation in accordance with hospital and regulatory standards
• Ensure compliance with state and facility guidelines
