Inspired Every Day. 6-8 hours, Monday through Friday Normal Business Hours Starting Pay Rate: $38.00/hour The Licensed Master Social Worker (LMSW) for the supportive housing program works under the supervision of the Supportive Housing Program Manager. This position focuses on providing clinical and case management support services to adults aged 55 and older who are living in permanent supportive housing and have histories of mental health and/or substance use disorders.

Requirements

  • Master’s Degree from an accredited college or university and LMSW in good standing in New York State required.
  • Minimum 1-year experience working with adults in mental health and/or substance use disorders, preferably in a supportive housing or community-based setting.
  • Basic computer skills are necessary including but not limited to the ability to work on a laptop or personal computer, the ability to create, open and save electronic documents and emails, and the ability to navigate between multiple computer programs within a Windows OS required.
  • Ability to effectively communicate and work as a member of an inter-disciplinary health care team
  • Ability to interact courteously, compassionately and tactfully with staff, residents, family members, visitors, and the public.

Responsibilities

  • Maintains an active role in selection of ESSHI participants as well as aids in discharge of participants needing a higher level of care
  • Provide individualized support and case management to residents in accordance with ESSHI guidelines
  • Conduct psychosocial assessments, assist in developing service plans, and monitoring progress toward recovery and housing stability goals.
  • Conducts 1:1 and group meetings to ensure compliance and safety of participants in areas of personal care, money management, self-medication, socialization and DSS benefits.
  • Collaborate with interdisciplinary team members, including property management, peer specialists, and external providers.
  • Facilitate referrals to mental health, substance use, medical, and social services as needed.
  • Maintain accurate and timely documentation in compliance with agency and funder requirements.
  • Participate in team meetings, high risk meetings and trainings as scheduled.
  • Support residents in developing independent living skills and community integration
  • Follows guidelines of HIPAA to ensure confidentiality of each resident
