About The Position
The Licensed Master Social Worker (LMSW) provides social work services to patients in a hospital setting. The role involves supporting patients and families by addressing psychosocial needs, assisting with care coordination, and collaborating with interdisciplinary healthcare teams to improve patient outcomes.
Requirements
- Master’s Degree in Social Work (MSW) from an accredited institution.
- Active Licensed Master Social Worker (LMSW) license issued by the New York State Education Department (NYSED)
- Minimum 1 year of social work experience
- Basic Life Support (BLS) certification required.
- Knowledge of hospital-based social work practices
- Ability to assess patient psychosocial needs
- Strong communication and interdisciplinary collaboration skills
- Familiarity with patient discharge planning and care coordination
- Ability to work in a fast-paced healthcare environment
Nice To Haves
- Hospital or inpatient care experience preferred , particularly in medicine, surgery, or rehabilitation settings
Responsibilities
- Provide social work services for patients in inpatient units including medicine, surgery, and rehabilitation (med/surg)
- Conduct patient assessments to identify psychosocial, emotional, and support needs.
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
- Assist patients and families with discharge planning and transition of care.
- Provide counseling, crisis intervention, and resource referrals when needed.
- Maintain accurate documentation in accordance with facility policies and regulatory requirements.
- Monitor patient conditions including basic patient observations such as vital signs when required.
- Participate in interdisciplinary meetings and care planning discussions.
