About The Position

As our Social Worker at Yavapai Regional Medical Center , you’ll perform social work assessments and interventions for inpatients, outpatients, and emergency department patients. Working collaboratively with all healthcare team members, you will develop and implement treatment plans that support patient-centered care for individual patients and the medical community. Being dedicated to patients and family-centered care, you value the personal self-determination of your patients. To succeed in this role, you must manage multiple priorities simultaneously and engage in creative, compassionate, and ethical problem-solving.

Requirements

• 1-Year Post-MSW experience or Social Work internship in a clinical or medical setting
• LMSW license from the AZ Board of Behavioral Health Examiners
• Master's Degree from a School of Social work accredited by the Council of Social Work Education
• Ability to work independently as needed with a high degree of detail orientation
• Ability to work efficiently in a fast-paced environment with changing priorities
• Ability to work collaboratively with an interdisciplinary care team

Nice To Haves

• Minimum 3-Year Post-MSW healthcare experience

Responsibilities

• Perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations
• Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients, and families.
• Serve on hospital committees and develop and present in-services to Hospital staff.
