Concordia at Cabot is seeking a Licensed Practical Nurse (LPN) – Discharge Planner to join our skilled nursing team. This role plays an important part in ensuring residents transition safely and smoothly from our facility to home or another level of care. If you are organized, collaborative, and enjoy coordinating care while working with a supportive leadership team, this is an excellent opportunity. This is a Monday through Friday position - 8:30 AM - 5 PM with only on call weekend coverage every four to six weeks. Pay: $30.00 - $33.00 per hour

Requirements

  • Licensed Practical Nurse (LPN) – active license required
  • Strong communication, teamwork, and organizational skills
  • Excellent critical thinking and problem-solving abilities
  • Ability to collaborate with multiple departments and providers
  • Compassionate approach to resident and family care

Responsibilities

  • Coordinate and manage resident discharge planning
  • Work closely with the DON, ADON, Therapy team, and interdisciplinary team (IDT)
  • Participate in care conferences and discharge planning meetings
  • Contact insurance providers to obtain authorizations
  • Arrange durable medical equipment (CME), home services, and prescriptions
  • Communicate with residents, families, and providers to ensure a smooth transition of care
  • Assist with mealtime support when needed
  • Participate in the on-call rotation with the management team
