About The Position

Northern Float : Toledo, Tiffin, Bryan, Defiance Licensed Practical Nurse (LPN) - Float Full Time Position No Weekends, No Holidays About Us Health Partners of Western Ohio is an independent, non-profit and community-directed organization. We serve low-income areas and places without access to care. We’re led by a volunteer Board of Directors. Most of our board members are also patients. Our Mission is to eliminate gaps in health outcomes for all members of our community by providing access to quality, affordable, preventive and primary health care. Join our Team! We are seeking a Licensed Practical Nurse who is adaptable, detail-oriented, and confident in managing both routine and complex care situations within our patient centered medical home setting. The ideal candidate will work closely with our medical provider team to deliver high quality, accessible care to our diverse patient population. You will apply your clinical expertise to perform a variety of complex nursing tasks and duties utilizing the nursing process. Requirements

  • Graduate of a school of nursing accredited by National League of Nursing.
  • Current license to practice as a Licensed Practical Nurse in the State of Ohio.
  • CPR – BLS (basic life support) required.
  • Ability to work as a team member.
  • Computer experience desirable.
  • Ability to work with a culturally diverse group of people.

Nice To Haves

  • One to three years full-time experience in nursing practice is preferred.

Responsibilities

  • Collects patient data through physical examination techniques, interview, and observation.
  • Documents all nursing care provided according to policy.
  • Formulates plan of care for clients under direction from providers.
  • Obtains referrals from other physicians and specialists.
  • Administers medications as prescribed by the provider.
  • Follows approved protocols/standing orders for client care.
  • Follow universal precautions.
  • Performs treatments, procedures as prescribed by the provider.
  • Performs technical skills.
  • Responds to client/family/caregiver needs.
  • Responds to crisis situations according to policy.
  • Utilizes client education for emotional and physical health maintenance.
  • Utilizes supplies in a cost-effective manner.
  • Performs clinic and outreach related tasks.
  • Utilizes on-going data collection skills.
  • Participates in Quality Improvement activities.
  • Demonstrates effective communication skills.
  • Establishes and maintains professional working relationships with other members of the health care team.
  • Participates in planning and problem solving.
  • Participates in continuing education and professional growth.
  • Attends staff meetings.
  • Maintains knowledge of, and complies with, personnel, nursing, medical, and clerical policies and procedures.
  • Assists with seasonal programs as needed.
  • Assists other team members.
  • Performs nursing visits.
  • Completes patient health screenings including hearing, vision, and all CLIA waived testing.
  • Maintains stocks of medicines and medical supplies as necessary.
  • Educates and advises patients on specified medical issues within established parameters.
  • Performs patient education and discharge instructions.
  • Completes referrals and schedules tests for patients.
  • Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG, guidelines, and other State and Federal laws). This also includes reporting to the Board of Directors, Compliance Officer, Privacy Officer, supervisor or suggestion box, any suspected unethical, fraudulent, or unlawful acts or practices.
  • Performs other duties as assigned.
