We offer a full continuum of services—including independent living, assisted living, skilled nursing, and memory care—designed to support residents at every stage of aging. We are seeking dedicated Home Care Licensed Practical Nurses (LPNs) who share our passion for helping older adults maintain independence, dignity, and quality of life. Our LPNs play a key role in supporting residents in both independent living and assisted living settings, providing high‑quality, person‑centered care that allows individuals to thrive in the place they call home. In this role, you’ll deliver direct patient care through established clinical practices, including administering medications and treatments, completing assessments, monitoring vital signs, and supporting diagnostic needs. Most importantly, you’ll bring genuine respect, patience, and compassion to the seniors who rely on you each day. Licensed Practical Nurse (Home Care) Under the direction of a Registered Nurse, provides home care patient care activities and accommodation services in accordance with standards, regulatory guidelines and licensure requirements. Maintains compliance with Agency policies and procedures. Meets, maintains and supports the standards of nursing care and practice in order to meet the healthcare needs of assigned residents.

Requirements

  • Has graduated from an accredited school of nursing
  • Current Nursing License
  • CPR certified
  • Minimum of two years work experience as a registered nurse required.

Nice To Haves

  • prefer at lease one (1) year of experience in a geriatric setting.
  • One (1) year home care experience preferred.
