Licensed Practical Nurse Intake Coordinator

About the position

Sentara is hiring a FLEXI remote LPN Intake Coordinator based out of Chesapeake, VA. The role involves evaluating and processing referrals by receiving and responding to incoming calls from referral sources or potential patients. The coordinator will record the outcome of calls and make follow-up calls as necessary, enter referral information in the system including clinical information and orders for services, and develop and maintain a working knowledge of all services and resources provided by Sentara and available within the community. The coordinator will act as a resource for patient and referral source issues.

Responsibilities

• Enter referral information in the system including clinical information and orders for services.
• Develop and maintain a working knowledge of all services and resources provided by Sentara and available within the community.
• Act as a resource for patient and referral source issues.

Requirements

• High school graduate or equivalent.
• LPN certification.
• Minimum 1 year of experience with clinical and customer service in a healthcare environment.
