About The Position

The Licensed Practical Nurse (LPN) provides direct patient care under the supervision of a Registered Nurse (RN). The LPN assists with treatments, supports patient assessments, and collaborates with the interdisciplinary team to ensure safe and effective care aligned with each patient's individualized plan.

Requirements

  • Graduate of an accredited Licensed Practical/Vocational Nurse (LPN/LVN) program
  • Minimum six months of Medical/Surgical experience in an acute care setting preferred
  • Current state licensure as a Licensed Practical/Vocational Nurse
  • CPR/BLS certification required
  • Strong clinical and patient assessment skills
  • Commitment to delivering safe, patient-centered care
  • Ability to collaborate effectively within an interdisciplinary healthcare team
  • Strong communication and patient education skills

Nice To Haves

  • IV Certification strongly preferred or must be obtained within 6 months of hire

Responsibilities

  • Administer medications safely and accurately based on patient age and clinical condition
  • Follow proper medication administration procedures to minimize the risk of errors
  • Assist the RN with initial patient assessments and reassessments in accordance with policy
  • Identify and implement appropriate rehabilitation nursing interventions
  • Complete Quality Improvement (QI) assessments accurately and on time under RN supervision
  • Deliver patient care based on response to treatment and update care plans as needed
  • Assess and reassess pain levels and implement appropriate pain management strategies
  • Educate patients and families on treatment plans and recovery expectations
  • Perform all patient care duties while promoting patient safety and reducing medical errors
  • Develop individualized patient and family teaching plans
  • Perform additional duties as assigned
