About The Position

The Licensed Practical Nurse (LPN) provides direct patient care under the supervision of a Registered Nurse (RN). The LPN assists with treatments, supports patient assessments, and collaborates with the interdisciplinary team to ensure safe and effective care aligned with each patient's individualized plan.

Requirements

Graduate of an accredited Licensed Practical/Vocational Nurse (LPN/LVN) program
Minimum six months of Medical/Surgical experience in an acute care setting preferred
Current state licensure as a Licensed Practical/Vocational Nurse
CPR/BLS certification required
Strong clinical and patient assessment skills
Commitment to delivering safe, patient-centered care
Ability to collaborate effectively within an interdisciplinary healthcare team
Strong communication and patient education skills

Nice To Haves

IV Certification strongly preferred or must be obtained within 6 months of hire

Responsibilities

Administer medications safely and accurately based on patient age and clinical condition
Follow proper medication administration procedures to minimize the risk of errors
Assist the RN with initial patient assessments and reassessments in accordance with policy
Identify and implement appropriate rehabilitation nursing interventions
Complete Quality Improvement (QI) assessments accurately and on time under RN supervision
Deliver patient care based on response to treatment and update care plans as needed
Assess and reassess pain levels and implement appropriate pain management strategies
Educate patients and families on treatment plans and recovery expectations
Perform all patient care duties while promoting patient safety and reducing medical errors
Develop individualized patient and family teaching plans
Perform additional duties as assigned
