Requirements

• Current, active LPN license in Georgia
• Minimum [1–2] years of nursing experience (home care experience preferred)
• Strong assessment and clinical documentation skills

• Ability to work independently and manage time effectively
• Valid driver’s license and reliable transportation

Responsibilities

• Conduct initial and ongoing in-home client assessments
• Evaluate client health status, functional abilities, and care needs
• Develop and update individualized care plans
• Document assessments accurately and in a timely manner
• Communicate findings with caregivers, families, and interdisciplinary team members
• Ensure compliance with agency policies, state regulations, and standards of care
• Participate in quality assurance and follow-up visits as needed
