About The Position

Licensed Practical Nurse (LPN) We’re looking for an LPN who puts compassion, dignity, and person-centered care at the heart of their work. Schedule: Monday through Friday, with normal business hours, with flexibility to adjust hours earlier or later based on needs of the individuals or scheduled appointments. Location: Faribault, MN Wage: $25.00 to $27.00 per hour depending on experience.

Requirements

  • Graduate of an accredited school of nursing; holds a current Minnesota Nursing License.
  • This position requires a valid Driver’s License and pass a driving record check conducted by our insurance carrier unless otherwise noted.
  • Excellent verbal and written communication skills, including the ability to effectively communicate with individuals and coworkers.
  • Must be self-motivated and exhibit superior interpersonal communication skills.
  • Display a positive attitude towards the job.
  • Must have a basic knowledge of technology programs. IE: Microsoft Office products.
  • Be able to use and/or be willing to learn technology for daily charting.
  • Be a team player with the ability to work independently and carry out job duties as assigned.
  • Display exceptional documentation skills, and knowledge of systems, routines, and programming.
  • Must pass the MN Department of Human Services Background Study.

Responsibilities

  • Provide compassionate, respectful nursing support to individuals in community-based and residential environments.
  • Deliver nursing care in accordance with LPN licensure, organizational policies, and individual service plans.
  • Support, coach, and educate direct care staff to promote safe, consistent, and high-quality care.
  • Ensure medical records and health documentation are accurate, complete, and maintained according to regulatory standards.
  • Guide staff in recognizing signs and symptoms of illness and in providing appropriate in-home care.
  • Complete required documentation, including incident reports, program reviews, psychiatric medication monitoring, and daily health logs.
  • Participate in onboarding, training, and ongoing coaching of staff to strengthen clinical knowledge and confidence.
  • Monitor medication administration practices, track medication errors, and collaborate with leadership on corrective actions and retraining.
  • Serve in the on-call rotation as scheduled to support individuals and programs after hours.
  • Communicate regularly with physicians, pharmacies, and other healthcare professionals to request orders, report concerns, and clarify medical needs.
  • Provide hands-on training to staff in medication administration, CPR, illness recognition, and other health-related topics.
  • When providing direct care support, assist individuals with activities of daily living (ADLs), leisure and community activities, household tasks, and medication administration in alignment with care plans.
  • Uphold HIPAA compliance and maintain confidentiality in all communications.
  • Follow all health, safety, and infection control policies and procedures.
  • Maintain an active LPN license, CPR certification, and pursue ongoing professional growth.
  • Perform additional duties as assigned, including administrative or program support tasks.
