About The Position

The Licensed Practical Nurse (LPN) at RHD is responsible for overseeing all aspects of medical care for individuals served by the organization. This role is part of a multidisciplinary team providing holistic, values-based services to promote integrated physical and mental health wellness.

Requirements

Licensed Practical Nurse (LPN) license.
Current license to practice in the State of Delaware.
Valid driver’s license.

Comfortable working in varied conditions (e.g., exposure to heat, cold, dust, fumes, confined spaces).
Physical abilities include standing, sitting, walking, climbing, reaching, kneeling, and repetitive motion.

Responsibilities

Manage medication ordering, delivery, storage, and administration per program guidelines.
Educate individuals and staff on health concerns; provide staff training as needed.
Assess and respond to new or ongoing health issues.
Complete required documentation for RHD and regulatory bodies.
Conduct screenings for depression, trauma, substance use, and criminal risk using validated tools.
Provide recovery-oriented, evidence-based treatment and collaborative documentation.
Act as liaison for medical care and community supports.
Coordinate care with prescribers and other providers; administer IM injections per physician orders.
Monitor vital signs and medication side effects; report findings to the team.
Ensure consumers have primary care providers, annual physicals, dental exams, and prescription schedules.
Participate in on-call rotation for 24-hour emergency and crisis intervention.
Develop recovery plans and recommend health objectives.
Deliver culturally competent, individualized services.
Perform other duties as assigned by the Team Leader/Program Director.
